During this 75-minute consultation, we will take the time to understand your current situation, talk through your most immediate concerns, and bring clarity to what comes next. Together, we will identify your priorities and create a practical, organized path forward.

Following the consultation, you will receive a personalized written action summary outlining recommended next steps and additional support options, when appropriate.

Ongoing Healthcare Navigation & Family Coordination — Starting at $175 per hour
Managing appointments, providers, family communication, and follow-up responsibilities can quickly become overwhelming. We provide calm, personalized support to help you and your family stay informed, organized, and connected throughout the process.

Support may include coordinating appointments and referrals, following up with providers on administrative matters, organizing healthcare information, maintaining care calendars and task lists, supporting family communication, and helping long-distance family members remain involved.
​
A two-hour minimum applies.

Appointment Preparation & Accompaniment — Starting at $350
You do not have to prepare for or attend an important healthcare appointment alone. We can help you organize questions and documents, coordinate transportation through a third-party provider, and meet you at the appointment for non-clinical support.

With your authorization, we can take notes during the visit and help organize the follow-up tasks afterward, so you leave with a clearer understanding of what needs attention next.
​
The starting price includes up to two hours. Additional time is billed at the applicable hourly rate.

Hospital-to-Home Transition Support — Starting at $1,750
Leaving a hospital or rehabilitation setting can bring relief, but it can also bring many new responsibilities and questions. We help make the transition home feel more manageable by organizing important information and coordinating the practical details.

Support may include reviewing and organizing discharge information, coordinating follow-up appointments, keeping authorized family members informed, arranging third-party transportation and other services, preparing the home for the client’s return, and creating a written transition action plan.
Preparing for a procedure involves much more than the appointment itself. We provide thoughtful, non-clinical coordination before and after the procedure so clients and families can focus more fully on preparation and recovery.

Support may include creating a preparation checklist, coordinating transportation and appointments, arranging meals or deliveries, organizing household support, keeping authorized family members informed, and tracking important recovery-related tasks.

This service may support clients preparing for outpatient procedures, cosmetic procedures, fertility-related procedures, and other planned treatments.
Procedure & Recovery Coordination — Starting at $1,500
Customized Navigation Packages — Pricing Provided After Consultation
Some healthcare situations require a more personalized level of planning and coordination. Customized support is available for transplant-related logistics, fertility treatment navigation, long-distance family coordination, and other complex healthcare circumstances.

Following the consultation, EliTzi Services will prepare a written service proposal based on the client’s needs, timeline, location, and desired level of support. This allows the service plan and pricing to reflect the client’s individual situation rather than using a one-size-fits-all package.
